Responsibilities
- Deliver physical therapy services to patients in their homes, including evaluation, diagnosis, and treatment planning within the home health framework.
- Develop and implement individualized care plans focused on functional mobility, gait, balance, strength, and pain management; adjust plans based on patient progress.
- Provide therapeutic exercises, manual therapy, and modalities as clinically appropriate; recommend assistive devices and safety adaptations for home environments.
- Educate patients and caregivers on exercises, positioning, safety, and home exercise programs; coordinate discharge planning and progression with the broader care team.
- Maintain accurate, timely documentation of assessments, interventions, goals, and outcomes in compliance with relevant regulations and payer requirements; ensure billing procedures are followed per organizational standards.
