Moms In Motion - A Virginia Service Facilitation Provider
Moms In Motion is seeking a Service Facilitator (SF) Case Manager to visit families at their homes and help them navigate Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT). You’ll support independent, safe living for clients and work remotely from a laptop when not on assignments. We value compassion, organization, and community focus.
