Assess patients, develop and implement individualized care plans, and monitor outcomes. Perform age-appropriate nursing skills and respond to changing patient conditions. Document patient care accurately in Epic. Provide patient/family education and discharge support. Collaborate with physicians and other healthcare professionals.
