RN Visit Nurse Overview As an integral member of our team, you'll: Perform comprehensive assessments to create and update nursing diagnoses and plans of care for home health patients. Administer skilled nursing procedures such as wound care, medication management, assessments, and treatments as ordered by physicians. Educate patients and their families on healthcare topics and self-care management to promote optimal wellness. Collaborate with multidisciplinary teams to coordinate overall plans of care and ensure holistic patient support. Maintain compliance with healthcare regulations, standards, and ethics while documenting assessments and treatments accurately. Participate in continuing education to stay current and maintain professional certification, adapting quickly to changes in the healthcare landscape. Qualifications To excel in this role, you'll need: A current Registered Nurse license in the state of Minnesota, along with a valid Driver's License and reliable vehicle. Knowledge of nursing theory and practice, strong organizational and interpersonal skills, and the ability to adapt quickly to changes and prioritize multiple tasks. Experience with Electronic Medical Records (EMR) software is preferred, and prior nursing experience in a home health, hospice, or TCU setting is ideal.
