Job Title: Case Manager RN

License: Nursing license required

- Experience with discharge planning of medically complex pediatric patients
- Bachelor's degree required
- Ability to work occasional weekends

Description:
We are seeking experienced nurses with a strong background in case management.
Candidates must possess at least 3-5 years of nursing experience, focusing on pediatric case management.
Experience with discharge planning for medically complex pediatric patients is essential.
A bachelor's degree is required for this position.
Occasional weekend work may be requested.
A case management certification is preferred.
