The Community Milwaukee, WI Milwaukee is the largest city in the state of Wisconsin and the fifth-largest city in the Midwestern United States. The city is located on Lake Michigan's southwestern shore and was incorporated in 1846. Milwaukee had a population of 590,157, making it the 31st largest city in the United States and the fourth-largest city situated along one of the Great Lakes. Major new additions to the city in the past two decades include the Milwaukee Riverwalk, the Wisconsin Center, Miller Park, The Hop (streetcar system), an expansion to the Milwaukee Art Museum, Milwaukee Repertory Theater, and Pier Wisconsin, as well as major renovations to the UWMilwaukee Panther Arena. The Client Forum opened in late 2018 and hosts sporting events and concerts. Since 1968, Milwaukee has been home to Summerfest, one of the largest music festivals in the world. With regard to education, Milwaukee is home to UW-Milwaukee and Marquette University. The city is home to two major professional sports teams, the Bucks and Brewers. It is home to several Fortune 500 companies, including Northwestern Mutual, WEC Energy Group, Rockwell Automation, and Harley-Davidson.

The Facility Milwaukee VAMC The Clement J. Zablocki VA Medical Center is located on 245 acres on the western edge of Milwaukee and part of VA Integrated Services Network 12 (VISN 12), which includes facilities in Iron Mountain, Michigan; Tomah and Madison, Wisconsin, and North Chicago, Hines, and Chicago in Illinois. The Medical Center delivers primary, secondary, and tertiary medical care in 196 hospital beds, and provides more than 857,000 visits annually through an extensive outpatient program. The nursing home care unit of 113 beds offers geriatric programming and the 150 domiciliary beds are the focus of programs in Substance Abuse Rehabilitation, Psychiatric Rehabilitation and Post Traumatic Stress Disorder care.

Essential Duties and Responsibilities The RN will serve as a Community Care Nurse Coordinator and will utilize their broad based clinical knowledge to manage basic, moderate, and complex care of the Veteran and the seamless transition to and from the community and to serve as a liaison to both internal and external Providers. The Community Care Nurse Coordinator functional role includes organizing and coordinating clinical components for the authorization of non-VA services in response to individual healthcare needs along the illness and care continuum and in multiple settings. Duties included but not limited to:

• Supports the Veterans referred for evaluation, coordinates between the Medical Center Providers and community Providers, and ensures appropriate follow-up consults are entered by the Provider.
• Coordinates patient appointments, conducts follow-up, reports test results to the appropriate Providers, and obtains records and test results from outside facilities
• Collaborates effectively with all levels of the health care team. This includes, but is not limited to clinics, inpatient departments, physicians, nurses, and other health care providers such as pharmacists, social workers, psychologists, physical therapists, dieticians, other outpatient departments, ancillary services, community Providers, etc
• Ensures instruction to the patient and the family is based on identified learning needs. Assesses the family's knowledge base, health status expectations, and the potential for the family member acting as the primary caregiver if necessary
• Assists with the development of activities and methods to ensure information is articulated and disseminated to staff
• Provides tracking and trending of referrals related to authorizations, billing, and clinical outcomes
• Documents patient's progress on clinical pathways/algorithms
• Plans care based on patient needs and health care needs, at home and in the health care setting
• Utilizes nursing interpretations to alert physicians to significant changes or abnormalities and provides medical information about the patient's condition, medical history and specialized treatment plan or protocol to other professionals.
• Applies critical thinking and analytical skills to identify barriers for care delivery

Requirements:

• ASN or BSN from an ACEN or CCNE accredited Nursing program.
• Current and unrestricted license in a State, Territory, or Commonwealth of the United States or the District of Columbia
• Evidence of completion of required licensure, credentials, required training, current competencies and fingerprints/background investigations must be validated

Shift: Minimum 8-hour shifts (typically 7:30am-4:00pm including a 1/2 hour non-paid lunch) Mon-Fri.
