Registered Nurse (RN) Hospice Home Health Case Manager
We are seeking a compassionate and experienced Registered Nurse (RN) Hospice Case Manager to join a dedicated hospice team serving patients in the Burlington and Walworth communities.
The RN Case Manager will coordinate and manage care for patients with end-stage illnesses, ensuring comfort-focused care in various home-based settings. This role requires strong clinical judgment, excellent communication skills, and the ability to provide holistic care to patients and their families.
End-stage hospice patients
- Care provided in:
- Patient homes
- Assisted Living Facilities (ALF)
- Skilled Nursing Facilities (SNF)
- Group homes
- Independent living communities
- Two hospitals providing General Inpatient (GIP) hospice care
Average Daily Census: ~105 patients
RN Case Managers: 7 (each managing 1416 patients)
CNA Support: 6 CNAs
- Manage a caseload of hospice patients in the community
- Provide comprehensive nursing assessments and symptom management
- Develop and implement individualized care plans
- Coordinate care with physicians, hospice staff, and interdisciplinary teams
- Educate patients and families regarding disease progression and end-of-life care
- Document patient care accurately in the EMR system
- Provide compassionate support to patients and families during end-of-life care
- Minimum Experience: 2 years RN experience
- Experienced travelers preferred
- Active Registered Nurse (RN) license
- BLS (Basic Life Support)
- Valid Driver's License
- Auto Insurance
- Reliable transportation for community visits
- Hospice or end-of-life care experience preferred
- Strong patient assessment and care coordination skills
- Ability to work independently in a community-based setting
- Excellent communication and compassionate patient care
- EMR: Epic & Rover
- IV Pump: Yes
- Medication Dispensing: Yes
- Monitoring Equipment: Chair and bed alarms
- Supplies: Home health bag and trunk supplies
- Shift: Monday Friday, 8:00 AM 4:30 PM
- Weekend Requirement: None
- On-Call: None
- Holiday Requirement: None
- Floating: Community-based patient home visits
- Orientation: 1 week
- Scrub Color: Blue or Black
- Parking: Free parking available at Burlington and Walworth offices
