Per Diem NP/PA for Long-Term Care Transitions

Optum is seeking a Per Diem Nurse Practitioner or Physician Assistant to support patients in skilled nursing and transitional care within the Patient Connect Program. You will perform one-time comprehensive health assessments in collaboration with care teams and case managers. The schedule is per diem with a minimum of 10 hours per week, offering flexible days, evenings, and weekends.
