Schedule: Monday–Friday (No weekend rotation)

A premier nursing and rehabilitation community in Middle Georgia is seeking a dedicated Physical Therapist (PT) for a permanent position. This role offers a consistent weekday schedule with no weekend requirements, supporting strong work-life balance while providing high-quality rehabilitative care.

### Key Responsibilities
* Evaluate and develop individualized treatment plans for residents
* Provide therapeutic interventions to improve mobility, strength, and function
* Monitor patient progress and adjust care plans as needed
* Collaborate with interdisciplinary teams to ensure optimal outcomes
* Participate in care planning and discharge planning
* Maintain accurate and timely documentation
* Educate patients and families on therapy programs and home exercises

### Qualifications
* Active Physical Therapist (PT) license in the state of Georgia
* Previous experience in skilled nursing, rehab, or long-term care preferred
* Strong clinical and communication skills
* Commitment to patient-centered care
