MDS Coordinator (LPN)

Agawam West Rehab & Nursing is seeking an experienced LPN to join our team full-time as an MDS Coordinator . This salaried position is responsible for coordinating resident assessments, supporting care planning initiatives, and helping ensure accurate reimbursement and regulatory compliance. The ideal candidate is organized, detail-oriented, and knowledgeable in the MDS process within a skilled nursing environment. Responsibilities Coordinate and complete MDS assessments and related documentation Ensure assessments are completed accurately and submitted timely Collaborate with nursing, therapy, and interdisciplinary team members Participate in resident care planning and assessment reviews Support reimbursement and regulatory compliance through accurate documentation Maintain current knowledge of MDS and CMS guidelines Qualifications Active Massachusetts LPN license required Previous MDS experience in a skilled nursing facility preferred Understanding of MDS scheduling, coordination, and completion Long‑term care or rehabilitation experience preferred Strong organizational and communication skills Compensation & Benefits Salary: $91,000 - $95,000 annually (DOE) Quality Measure Bonus: Up to $7,500 per year Salary increases tied to census Medical, dental, vision, 401(k), PTO, and more This is a direct hire position; we are not an agency. #J-18808-Ljbffr
