Key Responsibilities of the RN - Case Management

• Provide case management services for both inpatient and observation patients assigned.
• Perform comprehensive assessments of psychosocial, medical, and discharge needs of patients/family.
• Coordinate the plan of care and drive the discharge plan by collaborating with the multidisciplinary team.
• Evaluate progression of care using InterQual throughout the episode of care.

Requirements of the RN - Case Management

• RN License
• 2 years of Acute Case Management experience
• No call required
• 5 eight-hour shifts per week, rotating weekends
