You’ll build relationships, understand the challenges affecting their daily lives, and coordinate care around their individual needs. This role serves **Cleveland’s west side and nearby communities**, including **Lakewood, Rocky River, Westlake, North Olmsted, Avon, Avon Lake, Lorain, and Elyria**. **Why Choose This Opportunity?*** **One-on-one patient care** focused on individual needs.* **Work autonomy and flexible scheduling.*** Collaboration with a clinical team.* Opportunities to strengthen your skills through continuing education. **What You’ll Do*** Assess patients’ physical, behavioral health, psychosocial, and environmental needs.* Manage your assigned caseload, including assessing, planning, coordinating, providing, and evaluating nursing care.* Reassess patients during visits and update care plans as needs change.* Teach patients, families, and caregivers how to follow the care plan and manage symptoms.* Communicate significant findings, changes in condition, and safety concerns to the clinical supervisor, physician, and care team.* Coordinate with other providers and care settings to support continuity of care.* Supervise and document behavioral health aide and LPN care according to agency policy.* Maintain accurate clinical documentation.* Provide support at the time of death and complete bereavement assessments when applicable. **What You’ll Need*** Current, unrestricted RN license valid in Ohio.* At least **1 year of nursing experience in a clinical care setting**.* Ability and willingness to travel throughout the assigned territory, with approximately **50% travel**.* A dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.* Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment. **Preferred Qualifications*** Associate or bachelor’s degree in nursing.* Home health or hospice experience.* Homecare Homebase experience. **Benefits That Support You*** Competitive compensation.* Medical, dental, and vision insurance, plus pet insurance.* 401(k) with employer match.* Tuition reimbursement for full-time staff and continuing education opportunities for all employees.* Paid time off, paid holidays, and family and pet bereavement leave.*Benefit eligibility can vary by position and shift status.* **Ready to make your next nursing role more personal?
