Independent Contract Nurse Case Manager - Flexible Impactful

H&H Case Management and Consulting, PLLC in Los Angeles, CA, seeks an experienced Independent Contract Nurse Case Manager to provide patient-centered care management services. Work-life balance, autonomy, and flexible hours are emphasized as you coordinate care across hospital, home, and rehab settings. You will assess needs, develop plans, travel to patient sites within 100+ miles, and advocate for better outcomes.
