Remote LPN Care Coordinator - Chronic Care Management

CareHarmony’s Care Coordinators (LPN) work with providers to deliver value-based care management for their patients in a remote US-based setting.

The position requires an experienced LPN with at least 3 years of direct patient-facing work, strong attention to detail, and the ability to thrive in a fast-paced environment. You will identify resources and coordinate needs for chronic care management patients.
