Registered Nurse III-V - Public Health- CMS- Enhanced Care Management

Position Overview The County of Riverside, Riverside University Health System – Public Health, is recruiting for a Registered Nurse III-V in the Children’s Medical Services – Youth Enhanced Care Management department located in Moreno Valley. This program ensures that California Children Services (CCS) eligible children receive medically necessary, timely and coordinated care for children and youth with special health care needs and their families across Riverside County. Bilingual in Spanish is required. Responsibilities Provide comprehensive nurse case management services to Enhanced Care Management (ECM) California Children’s Services clients, including intake, assessment, care planning, service coordination, and transition support. Facilitate multidisciplinary case conferences with healthcare providers and manage care plans to address and resolve barriers to care and authorization requirements. Maintain timely and accurate documentation in Epic, monitor caseload performance metrics, and prepare materials for audits and compliance reviews. Educate families on care plans, medications, medical equipment, and navigation of complex healthcare systems. Support quality improvement initiatives and respond to escalated issues in a professional and timely manner. Travel countywide to partner sites and clinics; a valid California driver’s license is mandatory. Qualifications Experience: Three years of full‑time RN experience in an acute care facility, mental health facility, or community health agency; or a bachelor’s degree in nursing or a national nursing specialty certification may substitute for one year of experience; a master’s degree may substitute for two years. Registered Nurse IV: Six years of full‑time RN experience, with at least two years in a specialty area; a master’s degree may substitute for two years. Registered Nurse V: Option I – Nine years of full‑time RN experience, with at least two years in a specialty area; Option II – Seven years of full‑time RN experience, with at least two years in a specialty area; either option requires a bachelor’s or master’s degree in nursing. Strong working knowledge of California Children’s Services, Medi‑Cal managed care, and CalAIM Enhanced Care Management programs. Experience in pediatric nursing and care coordination. Required Licenses and Certifications Current California Registered Nurse license. Basic Life Support (BLS) certification issued by the American Heart Association. Public Health Nursing Certificate (obtainable within 6 months of hire). Certified Case Manager (preferred, not required). Valid California driver’s license. Salary Range Registered Nurse III: $47,893.34 - $71,229.50 Registered Nurse IV: $51,246.00 - $75,147.00 Registered Nurse V: $56,484.30 - $79,280.00 Schedule 9/80 schedule, with every other Friday off. #J-18808-Ljbffr
