Clinic Hours of Operation:

Monday - 7:30 a.m. - 6:00 p.m.

Tuesday - 7:30 a.m. - 6:00 p.m.

Wednesday - 7:30 a.m. - 6:00 p.m.

Thursday - 7:30 a.m. - 6:00 p.m.

Friday - 7:30 a.m. - 4:00 p.m.

• Columbus, Indiana: Just a short 25-minute drive to Columbus

• Louisville, Kentucky : Approximately one hour drive to Louisville

• Indianapolis, Indiana : Approximately one hour drive to Indianapolis

• Cincinnati, Ohio : Roughly 2-hour drive to Cincinnati

• Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) from an accredited school of medicine

• Completion of an accredited Residency Program

• Licensed or eligible for licensure in Indiana

• Board-Certified or Board-Eligible

• Active and unrestricted DEA license or ability to obtain prior to start

• Commitment to the mission of Indiana Health Centers, Inc.

• Prefer working at an FQHC, CHC, or PCMH
