Description The Family Interventionist provides comprehensive support to individuals and families in home, community, and clinic settings. This role is responsible for coordinating care plans, connecting clients with appropriate resources, and advocating for their needs to ensure continuity and quality of care. The Family Interventionist collaborates closely with interdisciplinary teams to deliver holistic, clientcentered services that promote positive developmental and behavioral health outcomes. All services are delivered with compassion, empathy, and adaptability, with careful consideration of each client’s unique needs and environment.
