Vivo HealthtStaff is seeking an experienced Registered Dietitian to work for a large geriatric organization in San Jose, CA.
Responsibilities:
1. Reviews diet orders in charts and confer with physicians, nurses, and patients.
2. Works with and participates actively in the interdisciplinary team regarding diet and nutritional problems and management, including patient care plans, problem-solving and case review.
3. Provides nutritional education to patients, family, and staff as needed; coordinates use of community resources.
4. Fulfills all requirements, such as reporting, keeping statistical records, making entries into charts and attending meetings as required.
Qualifications:
1. Graduate of an approved Dietetic program.
2. Registered by the Commission on Dietetic Registration.
3. 2 years experience as a dietitian focusing on care of the elderly or frail
4. California Driver's License
