Senior LTSS Service Care Manager (RN)

Position Title Senior LTSS Service Care Manager (RN) Work Location San Antonio and surrounding areas — Bexar County and counties that directly touch Bexar County. Assignment Duration 6 months with the possibility to extend or convert. Work Schedule 8:00 am – 5:00 pm, Monday‑Friday with the possibility for overtime. Work Arrangement Remote with field visits. Position Summary The STAR Kids Assessment Team completes initial assessments for children entering the STAR Kids program. Assessments determine unmet needs and identify Medicaid services that could benefit the child based on their condition. Background & Context Work is performed remotely from home. Multiple Teams chats are used for communication and questions. The Team Lead and other members are always available for support and promote an open communication environment. Key Responsibilities Complete 6 member visits per week, with 2 days allocated to documentation. Typically schedule 3 days with 2 visits per day and 2 full days for documentation. Travel from the employee’s home office to members’ homes and back for each visit. Schedule visits for the pediatric population at 3:00 pm, which may extend past 5:00 pm. Maintain heavy documentation duties with specific turnaround times for submission. Meet performance metrics measured by documentation accuracy, training compliance, and company goals. Qualifications & Experience Candidate Requirements Education/Certification (Required): RN ADN, BSN or MSN (Preferred) Licensure (Required): Registered Nurse (Preferred) Years of experience required More than 3 years of nursing experience and 2 years of case management experience. Disqualifiers None listed. Additional qualities to look for Home Health experience. Prefer candidates with pediatric experience.
