Our mission: “To make a positive difference in the lives of those we serve by providing competent, compassionate and outcome-oriented care – every day.” Job Title: Registered Nurse (RN) Location: Centerville Health and Rehab Job Type: Full-Time, Part-Time Salary: Negotiable Position Summary The Registered Nurse (RN) provides direct nursing care to residents and supervises day-to-day nursing activities performed by licensed practical / vocational nurses and certified nursing assistants in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures. Key Responsibilities Direct the daily activities of licensed practical/vocational nurses and certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures. Follow the nursing model to promote holistic care for residents. Ensure compliance with policies and procedures by assigned nursing personnel. Participate in admission, discharge and transfers of residents as required. Assess for changes in residents’ status, notify the physician and resident’s family or representative, and document accordingly. Transcribe physician orders to the medical record and carry out orders as written. Prepare and administer medications as per physicians’ orders and observe for adverse effects as indicated. Perform wound treatments as per physicians’ orders, observe for changes and document accordingly. Schedule follow‑up appointments for residents and transportation needs as indicated. Provide nursing leadership to nursing personnel assigned to the unit. Perform direct care skills such as colostomy changes, tube feeding, wound care, suctioning, IV administration, etc., in accordance with current policies and procedures. Perform rounds to ensure resident needs are being met and personnel are performing assigned duties. Collaborate with interdisciplinary team members as needed to ensure residents’ needs are holistically met. Participate in management team meetings to discuss census changes, resident status changes or care needs, or complaints or concerns. Other duties as assigned. Qualifications Prior experience in long‑term care preferred but not required. Degree in Nursing from an accredited college or university. Current unrestricted RN license in the practicing state.
