Job Type: Full-Time Schedule: Full-time, on-site leadership position About Oceana Healthcare Center Oceana Healthcare Center is a 116-bed skilled nursing facility located in West Los Angeles, including a sub-acute level of care. We are committed to providing resident-centered care in a professional, supportive, and interdisciplinary healthcare environment. We are seeking an experienced, organized, and compassionate Social Services Director to lead the facility’s Social Services Department and support residents, families, and the interdisciplinary team through admissions, care planning, psychosocial support, discharge planning, resident advocacy, and regulatory compliance. Position Summary The Social Services Director is responsible for planning, organizing, directing, and coordinating the facility’s social services program in accordance with federal and state requirements, facility policies, and resident-centered standards of care. This position works closely with residents, families, responsible parties, the Administrator, Director of Nursing, MDS, Case Management, Admissions, Business Office, Activities, Rehabilitation, physicians, outside agencies, and the interdisciplinary team to ensure residents’ psychosocial, emotional, discharge planning, and care coordination needs are identified, addressed, documented, and followed through in a timely manner. The ideal candidate is a strong communicator, highly organized, regulatory-aware, and able to lead with professionalism, accountability, compassion, and urgency in a fast-paced skilled nursing environment. Key Responsibilities Direct and manage the daily operations of the Social Services Department. Ensure residents receive medically related social services to support their highest practicable physical, mental, and psychosocial well-being. Complete timely psychosocial assessments, social services documentation, progress notes, care plan contributions, and discharge planning documentation. Participate in resident care conferences, interdisciplinary team meetings, discharge planning meetings, utilization discussions, and other resident-centered meetings as assigned. Coordinate and support resident discharge planning, including communication with residents, families, responsible parties, physicians, managed care plans, VA representatives, community agencies, placement resources, and other care partners. Assist residents and families with adjustment to facility placement, changes in condition, discharge planning, psychosocial concerns, advance directives, resident rights, grievances, and care-related concerns. Support compliance with resident rights, confidentiality, HIPAA, grievance follow-up, abuse prevention/reporting expectations, transfer/discharge requirements, and applicable SNF/sub-acute regulatory standards. Coordinate and oversee care conferences, family meetings, resident/family communications, transportation-related follow-up, and social services-related appointments as needed. Collaborate with MDS and the interdisciplinary team to ensure accurate psychosocial assessment, care planning, behavioral health documentation, discharge planning, and resident preference documentation. Support timely and appropriate communication with the Long-Term Care Ombudsman, community agencies, hospitals, managed care plans, VA partners, and other external entities as appropriate. Supervise, train, and support Social Services Department staff, including Social Services Coordinator, Social Services Assistant, or other assigned personnel. Maintain organized department systems, including care conference schedules, discharge planning tracking, grievance follow-up, appointment/transportation coordination, and social services documentation. Participate in facility quality assurance, compliance monitoring, audits, plans of correction, and performance improvement activities as assigned. Practice within the scope of education, training, and licensure, and refer residents to licensed professionals or appropriate outside resources when needs exceed the scope of the Social Services Department. Complete additional duties as assigned by the Administrator or facility leadership. Qualifications Bachelor’s degree in social work, psychology, sociology, gerontology, human services, healthcare administration, rehabilitation counseling, or a related human services field required. At least one year of supervised social work or psychosocial services experience in a healthcare setting, working directly with residents, patients, or clients, is required. Prior experience in a skilled nursing facility, sub-acute unit, long-term care facility, hospital case management department, behavioral health setting, or discharge planning role strongly preferred. Prior Social Services Director, Social Services Designee, Case Management, or SNF leadership experience preferred. Knowledge of skilled nursing facility operations, resident rights, discharge planning, care planning, psychosocial assessments, grievance follow-up, and interdisciplinary documentation preferred. Experience with PointClickCare or another electronic medical record system preferred. Strong written and verbal communication skills required. Strong organization, follow-through, documentation, and time management skills required. Ability to manage multiple priorities, meet deadlines, and work effectively in a fast-paced healthcare environment. Ability to communicate professionally with residents, families, responsible parties, staff, physicians, vendors, managed care representatives, VA partners, Ombudsman representatives, and outside agencies. LCSW, ACSW, or other applicable California behavioral health/social work credential preferred but not required unless required by the scope of assigned duties.
