As a Nurse Practitioner/Physician Assistant in Columbus, OH , you'll provide focused care in nursing facilities helping reduce readmissions and improve recovery while enjoying autonomy, a predictable schedule, and strong team support.


This is an onsite, multi-site position supporting three skilled nursing facilities (SNFs) in Columbus, Whitehall, and New Lexington, Ohio. The provider will work across all three locations based on patient and facility needs.

Flexible Schedule: Full-Time, Monday-Thursday; no call, no weekends

What You'll Do

• Round in assigned facilities on a consistent weekday daytime schedule

• Oversee daily patient interactions through timely micro visits that support focused assessments to assess recovery, manage pain, wounds, and functional decline
• Collaborate with physicians, therapists, and facility staff on individualized recovery plans

• Use Puzzle's EHR to complete documentation (often remotely from home)

• Monitor readmission risk and help close gaps in post-acute care

• Maintain compliance with post-acute and PM&R-related regulations

What You'll Gain

• Compensation: The anticipated total compensation range for this role is $120,000-$180,000 annually, inclusive of base salary and potential incentive and/or bonus opportunities. Final compensation will be based on experience, location, qualifications, and other lawful considerations.

Required Skills

-Master's degree in Nursing and current certification as a Nurse Practitioner (NP) from an accredited program.
-Active, unrestricted NP license in the relevant state(s) of employment, with prescriptive authority.
-Minimum of 2-3 years of experience in PM&R, geriatrics, post-acute care, or a related field; experience in nursing facilities strongly preferred.
-Strong clinical skills in assessing and managing barriers to recovery, including rehabilitation, pain, and chronic conditions.
-Excellent communication and time-management abilities to handle brief, efficient patient encounters.
-Proficiency with EHR systems and remote documentation.
-Ability to work independently with a high level of autonomy and consistency.
