About This Role As a Registered Nurse Case Manager at Elite Hospice of Mena, you will take full nursing responsibility for the delivery of the Plan of Care for each hospice patient, collaborating with other professionals to promote the well‑being of patients and their families. Highlights Pay: $71,640 - $107,460 annually Location: Mena, AR Brand/Division: Optum Schedule: Full Time What You’ll Do Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral. Identify the patient’s/family’s physical, psychosocial, and environmental needs, reassessing every 15 days. Initiate and coordinate the plan of care. Document problems, goals, interventions, and patient/family responses to hospice care. Collaborate with the patient/family, attending physician, and other members of the IDG for daily care. Instruct and supervise the patient/family in self‑care techniques when appropriate. Maintain accurate and relevant clinical notes regarding the patient’s condition. Function in any home situation regardless of client demographics. Requirements Education: Current and unrestricted RN licensure in the state of practice Experience: 1+ years of clinical experience preferred Licenses & Certifications: Current driver’s license, valid vehicle insurance, and access to a dependable vehicle, or public transportation; current CPR certification or ability to complete within 90 days of hire preferred #J-18808-Ljbffr
