Duties Description Reports to the Off Broadway Team TTL and serves as QIDP(Qualified Intellectual Disability Professional) for the Off Broadway Team.Instructs and supports individuals in developing and/or enhancing life skills necessary to live more independently in home and community settings; and develops, implements, and modifies habilitation plans to help individuals achieve their valued outcomes.• Collaborates with individuals, family members, advocates, care managers, and service providers to determine strategies for service delivery (e.g., skill acquisition/retention, staff supports, and/or exploration of new experiences); attends Life Plan meetings to assess the effectiveness of habilitation plans and services and establish objectives.• Assesses individuals’ skill levels in relation to their valued outcomes and identifies appropriate methods to teach and assist individuals in acquiring, improving, and maintaining life skills.• Communicates with treatment team members and gathers information to plan and/or modify services to meet individuals’ needs.• Implement appropriate safeguards and protective oversight measures to ensure the health and welfare of individuals while participating in programs and services.• Monitors, records, and measures individuals’ progress in reaching their goals, including accomplishments and challenges; and modifies habilitation plans to ensure continual progress, prevent regression, and meet individuals’ needs.• Schedules, transports, and assists individuals in participating in community.Additional duties include but not limited to:• Develops and updates Individuals Safeguard Summary Plans (SSP).• Develops and updates residential Staff Action Plans (SAP) with the individual, family/advocate, Program Planning Team, Care Coordinator and service providers.• Provide weekly/monthly review of service notes documentation for Medicaid Compliance Billing • Participate in the development and review of Support and Risk Accountability Plans (SRA). Present at SRA meetings.• Train residential staff on the implementation of Staff Action Plan outcomes and documentation of individual’s response to services.• Monitor the implementation of Safeguard Summary Plan (SSP), Staff Action Plan (SAP) and Support and Risk Accountability Plan (SRA).• Review all documentation, assess status and progress of the Staff Action Plan (SAP), Support and Risk Accountability Plan (SRA) and Safeguard Summary Plan (SSP). Revise as needed to assure that plans continue to reflect the individual’s abilities and need for protective oversight and supports. Review billing documentation monthly. • Submit and Present information for the Behavioral Intervention Rights Committee (BIRC).• Submit Clinic Referrals; Disseminate copies of completed clinic reports. • Participate in the Life Plan review meetings, House and Team Meetings, Special Meetings and take minutes.• Participate in Programmatic Self-Assessment and Regulatory Plans of Corrective Action (POCA) development, implementation and monitoring as relevant to the QIDP role.• Participate with other team members in a variety of individual, team and continuous improvement meetings and forums as scheduled to promote comprehensive and individualized services.• Must have the ability to adjust work hours to meet operational/supervisory needs including but not limited to Life Plan Meetings, trainings, and off shift visits to the houses on the team.• Complete additional tasks, as assigned by the TTLLinkedIn:

Minimum Qualifications Competitive Minimum Qualifications:Must be on the Civil Service eligible list for this location. Transfer Qualifications: Must be in title or eligible for transfer under Section 70.1 in a designated title determined by the New York State Department of Civil Service. For eligible 70.1 information, visit the Career Mobility Office at: doing this search use your current title to see the titles you can transfer to.Non-Competitive Minimum Qualifications:A Bachelor’s or higher-level degree in a human services field* *For the purpose of these qualifications, a degree in a human service field includes the professional disciplines of audiology, community mental health, nursing, nutrition, occupational therapy, physical therapy, psychology, rehabilitation counseling, vocational counseling, social work, speech/language pathology and therapeutic recreation (including concentrations in art, creative art, dance, drama, music, or physical education.) Also included are related academic disciplines associated with the study of human behavior (e.g., gerontology, human services, sociology, speech communication); or human skills development (e.g., counseling/guidance, education, human development.) Examples of non-qualifying degrees include but are not limited to: criminal justice, political science, liberal arts, and business.

Additional Comments PREFERRED QUALIFICATIONS: Qualified Intellectual Disabilities Professional (QIDP) Status.Preferred qualifications are at least one year’s experience providing services to persons with developmental disabilities AND either; a master’s degree in a field of psychology, a bachelor’s degree in a human services discipline, a psychology license currently registered with the NYS Education Department, a nursing (or licensed practical nursing) license currently registered with the NYS Education Department, or a master’s degree in social work with current certification and registration with the NYS Education Department as a social worker.
