CCHC is seeking a Care Coordinator to join the care management team. You will assist with outreach to patients, gather medical records, and support the care management model across the care team in accordance with CCHC policies. You will monitor MHN portal, conduct initial screenings, educate patients, and coordinate with Medical Home to develop individualized care plans, while addressing barriers to adherence and ensuring timely appointments. #J-18808-Ljbffr
