This place really enhances one's customer service skills and lets you build meaningful relationships with clients." — ComForCare RNWhat We OfferWork-life balance: Weekday day shifts with flexibility built around your life, family or continuing educationBenefits (30+ hours/week): Health, dental, vision, life insurance and short-term disability401(k) with company matchPaid time off (PTO)What You'll Do as an RN Case ManagerClient intake and assessments: Complete initial and ongoing in-home nursing assessments of each client's physical, cognitive, psychosocial and home safety needsCare plan development: Create and update individualized plans of care and make sure caregivers understand and follow themCaseload management: Oversee a caseload of home care clients, tracking changes in condition and adjusting services as needs changeCaregiver supervision and training: Conduct supervisory visits and competency checksCare coordination: Serve as the main clinical contact for families, physicians, hospitals and community partners; make referrals when neededDocumentation and compliance: Keep accurate, timely client records and follow Colorado home care regulations, infection control and HIPAA guidelinesOn-call support: Help resolve client and caregiver concerns quickly and professionallyQualificationsActive, unencumbered Registered Nurse (RN) license in ColoradoAt least 1 year of home care or home health nursing experience (required)Case management, care coordination or caregiver supervision experience (preferred)Graduate of an accredited school of nursingCurrent BLS/CPR certificationOasis DocumentationValid driver's license and reliable transportationStrong clinical judgment, organization and communication skills, plus a genuine passion for client-centered careReady to Lead Care Your Way?Apply today to connect with our team and see how rewarding home care case management can be.
