Growing together.The Physical Therapist - HAC is responsible for helping patients restore function, improve mobility, and enhance overall quality of life. This role provides skilled physical therapy services under a physician-directed plan of care, including assessment, treatment planning, intervention, and care coordination within an interdisciplinary care model.Primary Responsibilities:Conduct comprehensive patient assessments and develop individualized physical therapy plans of care in collaboration with physiciansProvide skilled physical therapy interventions and modify treatment plans based on patient progress and clinical needsPerform ongoing reassessments, including weekly evaluations, to support care planning and physician coordinationCollaborate with interdisciplinary teams to coordinate care and communicate patient progress and condition changesMaintain accurate, timely documentation in compliance with clinical, regulatory, and documentation standardsYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Graduate of an accredited physical therapy program (CAPTE accredited)Current, unencumbered license to practice physical therapy in the assigned stateDriver's License and access to reliable transportationCurrent CPR certification1+ years of experience in physical therapy; OR less than one (1) year with participation in an approved residency or mentorship program1+ years of experience in clinical assessment, documentation, and care coordinationPreferred Qualifications:1+ years of experience in home health, hospital-at-home, or community-based therapy settings1+ years of experience working within interdisciplinary healthcare teamsPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $76,608 to $114,912 annually based on full-time employment. We comply with all minimum wage laws as applicable.
