Location: Woodland Manor - Elkhart, IN
Schedule: PRN (As Needed)

Woodland Manor is seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team in Elkhart, IN. This flexible PRN opportunity is ideal for a skilled clinician who enjoys helping residents improve strength, mobility, balance, and independence while providing high-quality, patient-centered care in a supportive environment.

What You'll Do:

Evaluate residents and develop individualized physical therapy treatment plans
Provide evidence-based, patient-centered therapy interventions to improve mobility, strength, balance, and functional independence
Monitor resident progress, update goals, and modify treatment plans as appropriate
Collaborate with the interdisciplinary care team to support coordinated and effective rehabilitation outcomes
Maintain accurate documentation and ensure compliance with clinical and regulatory standards
Educate residents, families, and caregivers on exercises and strategies that support continued progress and independence

Qualifications:

Licensed Physical Therapist (PT) in the state of Indiana
Current BLS certification
Strong clinical judgment and communication skills
Compassionate, patient-focused approach to care
Ability to work independently while collaborating effectively with the rehabilitation and interdisciplinary teams
