Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment based on physician orders/referrals
• Complete comprehensive evaluations using proper assessment tools
• Review patient records for diagnosis and medical history
• Establish measurable goals and develop treatment plans
• Re-assess treatment effectiveness and modify as needed
• Communicate patient progress to the treatment team and relevant parties
• Consult with medical providers about patient progress and discharge plans
• Recommend and obtain adaptive equipment to enhance rehabilitation
• Maintain evaluation, treatment, and billing documentation as required
• Supervise Physical Therapist Assistants and Aides
• Attend and conduct staff development programs
• Maintain professional practice standards and comply with regulations

Requirements of the Physical Therapist

• Ga License (Compact license accepted)
• Proficiency in HCHB
• Previous home health experience
