Key Responsibilities\n- \n
- Conduct comprehensive patient intakes, focusing on diagnostic and functional evaluations, and make informed recommendations to primary care providers regarding treatment plans. \n
- Provide brief, focused therapeutic interventions for patients requiring mental health services, adapting techniques to meet individual needs. \n
- Monitor and coordinate behavioral health services, assist in the formulation of self-management plans, and follow up with patients to evaluate progress. \n
- Develop and implement relapse prevention strategies for patients at risk of exacerbating their conditions. \n
- Document patient progress meticulously in treatment charts, ensuring accuracy to support ongoing care. \n
- Collaborate with the primary care team to address mental health and psychosocial issues while advising on referrals for advanced mental health services as necessary. \n
- Educate patients on coping strategies, medication compliance, and lifestyle modifications that promote better health outcomes. \n
- Engage in ongoing program evaluation and take part in consultations with clinic management to improve behavioral services. \n
- Maintain adherence to the Code of Ethical Conduct for Clinical Professionals at all times. \n
The ideal candidate will work with a diverse range of clients, including individuals and families facing mental health challenges such as anxiety, depression, trauma, and adjustment disorders. The ability to engage effectively with clients from various backgrounds, including bilingual capabilities (English-Spanish), is highly regarded.
Qualifications\n- \n
- A Master’s Degree in Clinical Counseling from an accredited institution. \n
- A current and valid Licensed Clinical Professional Counselor (LCPC) license in the state of Illinois. \n
- A robust understanding of mental health assessment, treatment planning, developmental psychopathology, and psychotherapeutic techniques. \n
- Strong ability to create therapeutic relationships with clients and an understanding of community resources available for patient support. \n
- An ability to adapt therapies and interventions to address the psychosocial challenges that influence patient health. \n

- Experience in a primary care or behavioral health setting. \n
- Familiarity with DSM-IV diagnoses and an understanding of various psychotherapeutic modalities. \n
- Previous experience working collaboratively with a multidisciplinary healthcare team. \n
- Bilingual English-Spanish is preferred, enabling effective communication with a broader client base. \n
