Ambulatory Care Clinical Pharmacist — Telehealth/Hybrid

Cambridge Health Alliance is seeking a clinically skilled Clinical Pharmacist to join our ambulatory care team. You will provide direct patient care across adult primary care and specialty sites within a hybrid model (telehealth, in-person visits, home visits for select patients). Responsibilities include pharmacotherapy management, education, and collaboration with physicians and nurses to optimize therapies and outcomes.
