Requirements:

• 2+ years of experience as a PT
• Qualified applicants MUST have at least 2 years of experience in Home Health
• Valid Physical Therapist license
• Be willing to obtain South Carolina licensure

Responsibilities:
As a healthcare professional in a Home Health setting, you will deliver one-on-one, high-touch care to patients in their homes, conduct skilled nursing visits and assessments, create and manage individualized care plans, and collaborate with families and interdisciplinary providers to improve patient outcomes.

• Provide personalized physical therapy (PT) services to patients in their homes.
• Assess patient mobility and functional status, developing tailored treatment plans that promote recovery and independence.
• Utilize industry keywords such as patient-centered care, assessments, and therapy planning in your clinical approach.
