Your Role Perform patient evaluations and develop individualized treatment plans Provide PT services to short-term rehab & long-term care residents Improve patient mobility, strength, balance, and functional outcomes Document progress and outcomes accurately Collaborate with interdisciplinary care teams

Requirements Active California or Arizona PT license (dependent upon assignment state) Prior experience in a Skilled Nursing Facility Strong communication & documentation skills Ready to start ASAP
