Specific Responsibilities:
This is a Nursing Program Specialist position in the Healthy Start/Healthy Beginnings program providing in-home health education, support and case management, to improve pregnancy outcomes, child health and development and parental life course to a culturally diverse population of pregnant women or families with newborns in Palm Beach County. Works is performed under the supervision of a Senior Community Health Nursing Supervisor. This position requires independent judgment, initiative, applying general knowledge of business practices and skill in applying instructions to accomplish various job functions. Adherence to all established Florida Department of Health Safety Procedures and Practices while in the workplace and in the community.
This position is designated as a sensitive position and requires maintaining confidential information in accordance with the Palm Beach County Health Department (CHD) and Children’s Services Council Information Security Policy, Protocols and Procedures. Confidential data sets: Has access to include Clinical Information, Health Management System (HMS)-Employee Activities Record (EARS), Florida Shots, Healthy Start Clinic Information and Healthy Beginnings Data System (HBDS) for the purpose of entering and reviewing client data to view, update or release client information and uses an electronic medical record database.
Conducts home visits providing curriculum-based health education services to pregnant women and families with newborns in accordance with the Health Start Pathway and the Healthy Start Standards and Guidelines. Initiates the planning and implementation of education and needed services for individual clients and their families. Completes required Healthy Start Pathway Assessments and Educational Models. Reports concern to the client's health care provider as needed. Individualizes approaches used during client encounters to meet the unique needs of each family by engaging families as full partners in their own and their children’s development. Assesses client-learning styles and modifies teaching methods to meet individual needs. Builds families self-esteem by drawing on their cultural strengths to foster positive family interactions. Engages families to assess their own needs and strengths to develop and reach realistic and achievable long- and short-term goals. Builds and maintains therapeutic long-term relationships with families while maintaining professional boundaries. Strengthens the parent-child relationship by helping families engage with their children through daily routine experiences in the home and community.
Provides case management services during home visits or on behalf of program participants. Communicates with Healthy Beginnings System Partner Agencies and Health-Care Professionals to assure a coordinated approach to the care of the mother, child and family. Identifies community resources, makes referrals and links families with appropriate services as needed. Documents the provision of all client services: activity codes, progress notes, assessments, screenings, family support plans, referrals, immunizations, medical appointments, and other activities in the Health Beginnings Data System in accordance with the Healthy Start Standards and Guidelines and Children’s Services Council Grant requirements.
Participates in interdisciplinary team case conferences, reflective practice group sessions, individual reflective supervision, team meetings, agency meetings, and community/committee meetings. Completes mandatory trainings, daily activity logs, home visit forms, weekly client visit and assessment schedules, and monthly reports. Submits Statewide Travel Management System (STMS) travel authorizations and reimbursements.

- This occupation requires licensure as a Registered Professional Nurse in accordance with Chapter 464 Florida Statutes and in accordance with Rule 64B9, Florida Administrative Code.

• Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
• Valid Driver’s License
• Must have access to transportation.
• Travel throughout Palm Beach County is required.
• Out of state and/or out-of-county travel may be required for mandatory trainings and continued education.
• This position works primarily in a community setting providing home visits and involves getting in and out of a vehicle, walking up and down stairs, and moving, lifting, pushing and carrying objects up to 25 lbs. multiple times throughout the day.
Working hours: (A) Daily from 8:00 AM to 5:00 PM (B) Total hours in workweek 40 (C) Explain any variation in work (split shift, rotation, etc.)
- This position may be required to work and provide home visits before and/or beyond the normal core business hours of 8:00 AM to 5:00 PM and on weekends due to the needs of program participants.
- This Position requires in-office and field work.
