Licensed Practical Nurse Home Health

Overview Community Home Health Services, a division of Medical Services of America Inc., seeks an experienced Full‑Time Licensed Practical Nurse (LPN) for Home Health patients in Palm Beach County, FL. As part of the multidisciplinary team, the LPN provides selected nursing care under the direction of a registered nurse. Responsibilities Provides skilled nursing care to clients as directed by the registered nurse and physician’s plan of care in accordance with agency policies. Adherence to policies and procedures. Assists in planning and exchanging information with other health professionals to help achieve continuity of client care. Prepares clinical progress notes and follows client care plans as developed by the registered nurse and according to agency policy. Participates in on‑call rotation. Requirements Successful completion of a certified licensed practical nursing program. Currently licensed as an LPN in the state of FL. Minimum of one year of nursing experience. Home Health experience strongly preferred. Reliable transportation and company‑required auto liability insurance.
