About the Position

Job Title: Home Hospice Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (10:00 - 18:00) Experience: Must have experience with home hospice case management. License: VA RN License required. Certifications: BLS required. Must-Have: Experience with home hospice case management is essential. Candidates must have EPIC charting experience. Description: The position involves working as a home hospice case manager. The expected patient census for this role is approximately 35-45 patients. Candidates must possess the required certifications and licensing. Floating may be requested based on the needs of the facility. Specialty experience within the last year is required. Candidates must provide a supervisory reference for submission. All applicants must comply with the educational and training requirements. Any time-off requests must be kept to a minimum during the assignment.
