Home Health Aide

Responsibilities and Duties Work with a registered nurse and other clinicians to provide care based on an individualized plan of care. Record your patient's health status and report it to the nurse. Provide companionship and personalized care while encouraging independence and self-care. Assist patients with activities of daily living, including dressing, bathing, grooming, toileting, and eating. Provide mobility and transfer assistance, which may include using mechanical lift devices. Prepare meals and snacks. May include grocery shopping. Perform light housekeeping, including vacuuming, laundry, and linen changes. Assist with incontinence care. Assist with transportation needs. Qualifications and Skills Current and valid Home Health Aide certification CPR certification Ability to lift at least 50 pounds Availability to work on weekends, evenings, and holidays Reliable transportation Valid driver's license and insurance Prior home health experience preferred Participate in facility and company required trainings, in-services and conferences. Able to supervise and support physical therapist assistants; rehabilitation aides and/or students. Adherence to Company required policy and procedures and code of conduct.
