Sage Connections is seeking a dedicated and compassionate Registered Nurse (RN) to provide 1:1 support for a student in a school-based setting for the 2025–2026 school year in Elgin, IL.

Position Details

• Age Range: 1:1 Assignment
• Hours per Week: ~37.5
• Schedule:  Monday - Friday
• Start & End Date:  August 2026 - June 2027
• Onsite/Remote/Hybrid:  Onsite

Responsibilities

• Provide direct 1:1 nursing support throughout the school day.
• Assist with activities of daily living (ADLs) as needed.
• Monitor and manage students’ health needs, including medication administration, feeding support, and routine health checks.
• Support students with mobility assistance and transfers as required.
• Collaborate with school staff and families to ensure individualized care plans are followed.
• Maintain accurate documentation of nursing care and student health status.
• Respond to medical emergencies according to established protocols.

Qualifications

• Active and in-good-standing RN license (state-specific).
• Experience with pediatric nursing, school-based nursing, or similar settings preferred.
• Knowledge of common medical procedures such as medication administration, feeding tube care, and blood glucose monitoring.
• Current CPR certification.
• Strong communication, organizational, and clinical decision-making skills.
• Compassionate and student-centered approach to care.
