Key Responsibilities
• Provide clinical leadership and supervision to ensure patient care aligns with professional standards and regulatory requirements
• Oversee and coordinate patient care delivery across assigned territory
• Support, mentor, and guide clinical staff including RNs and field clinicians
• Monitor patient outcomes and ensure timely updates to care plans based on condition changes
• Collaborate with physicians, interdisciplinary teams, and internal leadership
• Ensure accurate, timely clinical documentation and compliance with organizational standards
• Participate in quality improvement initiatives and support achievement of performance goals
• Maintain strong communication with patients, families, caregivers, and staff
• Promote a safe, compliant, and high-performing care environment

Required Qualifications
• Active Registered Nurse (RN) license in the state of California
• Graduate of an approved school of professional nursing
• Minimum 3–5 years of RN experience required
• Prior experience in home health, community, or clinic settings preferred
• Current CPR certification required while employed in a clinical role
• Valid driver's license, reliable transportation, and active auto insurance required
• Strong clinical judgment, leadership, and communication skills
• Ability to travel within assigned territory and work independently in a field-based role

Pay Details: $125,000.00 to $145,000.00 per year
