Job Description
TITLE
LPN/LVN - Long-Term Care

SCHEDULE
Contract Length: 13 weeks
Shift: 5x8, 1500-2300 (Evening)

ASSIGNMENT DETAILS
Census/Beds: ~25

REQUIREMENTS
- Active New York single-state LPN license (no pending licenses)
- Minimum 1 year clinical experience
- Nursing facility/skilled nursing facility experience required
- Long-Term Acute Care (LTAC) experience required
- Previous charge experience required
- Travel experience required; first-time travelers accepted
- BLS certification required
- EMR: EPIC experience required
- Key clinical skills, including skilled nursing facility care, behavior management and de-escalation techniques, care for degenerative disorders (e.g., Alzheimer's, ALS, Parkinson's), obtaining 12-lead EKG/ECG, vital sign monitoring, pain assessment/management, restraint protocols

COMPLIANCE / NOTES
- Weekend requirement: Every other weekend
- Holiday requirement: Available every other holiday, if needed
- Must have active NY LPN license to submit
- Cannot submit candidates with more than 10 days RTO
- Candidates within 75 miles of facility: posting notes restriction (verify before submitting)
- Background policy: reviewed case-by-case
- Return-staff policy: 1 year
- Modules may be required pre-start
