Community Health Navigator & Advocate

Independence Health System

Independence Health System is seeking a Community Health Worker to serve as a trusted liaison, improving access to care and reducing disparities through outreach, education, and resource navigation. You will work with community partners and social service agencies to connect patients with services, advocate for care, and coordinate follow-up by phone and in person while maintaining accurate documentation and collaboration with providers. #J-18808-Ljbffr Independence Health System
