Licensed Master Social Worker - Nursing Home Surveyor/Complaint Investigator - Syracuse, NY (#25312A) Location: Syracuse, NY Employment Type: Full-time, Temp to Perm Hourly Rate: $39/hr Position Overview Based in Syracuse, NY, the Nursing Home Surveyor/Complaint Investigator will assist with the surveillance and investigation activities related to nursing homes (long‑term care). These activities evaluate facility performance and monitor the quality of care and services provided. The role participates as a member of an interdisciplinary survey/investigation team to review allegations of abuse and neglect, perform surveillance activities/tasks at nursing homes, Transitional Care Units (TCUs), and Adult Day Health Care Programs (ADHCP) to determine compliance with participation requirements; prepare written reports of investigative findings and Statement of Deficiencies (SOD) within required time frames using Principles of Documentation; participate in emergency surveys at Skilled Nursing Facilities (SNFs) and during Immediate Jeopardy state monitoring sessions; and perform other related duties. Technical Skills Strong interpersonal skills with the ability to facilitate positive customer satisfaction in adverse or controversial situations. Ability to work independently with minimal supervision, as well as in a team environment. Excellent written and verbal communication skills with the ability to relate effectively to providers, physicians, senior medical and administrative personnel, and residents/patients. Superior investigative and analytical skills with proficiency in conducting interviews with facility staff, residents, and families. Strong computer skills with the ability to learn new programs. Ability to obtain Surveyor Minimum Qualifications Test (SMQT) certification within 12 months of employment, required. Ability to travel to an on‑site location. Other Requirements Candidates must be flexible with scheduling, as the state will determine the work schedule. Employees must be available to work weekends, holidays, and evening shifts, as needed. Weekend, holiday, and evening shift schedules will be assigned on a rotating basis. This is a travel position. Candidates will receive a premium pay rate for working on holidays and overtime. Key Responsibilities Participate in surveys or complaint investigations, including allegations of abuse and neglect, and perform surveillance activities/tasks at nursing homes to determine compliance with federal and state regulations. Produce written documentation and draft a Statement of Deficiencies (SOD) within the required time frame, using Principles of Documentation. Assess compliance with the Plan of Correction within the required time frame. Complete data entry in federal and state reporting databases as required. Participate in state monitoring during immediate jeopardy situations. Attend meetings as required. Testify in administrative hearings as needed. Adhere to DOH work schedules.
