# Skilled Nursing RN/LPN
Pulaski Health Care Center is seeking an RN or LPN for a skilled nursing assignment in Winamac, Indiana. The position is based at 624 East 13th Street, Winamac, Indiana 46996.

Eight-hour mid-shift positions are available. Candidates must have at least two years of recent long-term care experience, BCLS certification, and an active Indiana RN or LPN license. Matrix experience is preferred, but training will be provided.

## Responsibilities
- Provide RN or LPN services within the long-term care setting.
- Complete assigned skilled nursing responsibilities during eight-hour mid shifts.
- Use Matrix after training if the candidate does not already have experience with the system.
- Meet the facility's listed screening and health documentation requirements.

## Qualifications
- Active Indiana RN or LPN license.
- At least two years of recent long-term care experience.
- Current BCLS certification.
- Indiana DHHS background check.
- National, state, and county background checks.
- Ten-panel drug screen.
- Two-step TB documentation and Hepatitis B, MMR, Tdap, and Varicella records.
- Matrix experience is preferred; training is available.
