Must-Have:
- Previous experience in acute care coordination

- Experience with discharge planning and utilization review
- Familiarity with Medicare/Medi-Cal guidelines related to skilled nursing facilities (SNF) and lower-level placements


Description:
The role requires a fast learner who is adaptable and flexible.
Candidates must demonstrate excellent attendance and a professional appearance.
Dress code is business casual and conservative, with no open-toed shoes or strong perfumes.
The position involves a 1:20 patient assignment ratio.
The employee will float across all adult areas of the facility.
Candidates need to have experience with heavily socially-impacted discharges, including room and board placements and conservatorships.
Experience working with placement agencies and discharge planning for under-resourced individuals is preferred.
The position involves coordinating care and managing complex discharge plans.
Candidates must be prepared to tackle challenging placement scenarios.
Familiarity with strategies for discharging individuals without heavy resources is essential.
