Position Overview Per diem and per assessment Advanced Practice Clinicians and Physicians are eligible for a $10,000 sign‑on bonus. As a team member of our Senior Community Care (SCC) product, you will provide care to patients at home in nursing homes, assisted living, or senior housing. This life‑changing work adds a layer of support to improve access to care and creates a seamless health journey across settings. The role focuses on patient transitions from skilled rehab facilities to home. You will capture STAR measures, educate the patient, and document all acute and chronic diagnoses. While you will not be managing care, you will communicate findings to attending and facility staff, allowing you to work independently with autonomy. Primary Responsibilities Perform comprehensive assessments and document findings in a concise and comprehensive manner that meets CMS documentation requirements. Ensure all diagnoses are ICD‑10 coded accurately and documented appropriately to support the visit. Ensure all quality elements are addressed and documented. Perform an initial medication review. Attend and complete all mandatory educational and computer/virtual training requirements. Travel between care sites as required. Care coordination: communicate with the patient and necessary team members regarding medical concerns at the time of the visit. Program Enhancement Expected Behaviors: Exhibit original thinking and creativity in developing new methods. Function independently with minimal supervision. Enter available hours into the web application at least one month prior to work time. Demonstrate initiative in achieving individual, team, and organizational goals. Participate in SCC quality initiatives. Check Optum email intermittently for required trainings, communications, and monthly scheduling. Required Qualifications Certified Nurse Practitioner through a national board. Graduate of an accredited master’s degree in nursing (MSN) program and board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC), adult‑gerontology acute care NP, adult/Family or Gerontology NP. Certified Physician Assistant through a national board; graduate of an accredited PA degree program and currently board certified by the NCCPA. Active and unrestricted license in the state in which you reside. Ability to lift a 30‑pound bag and navigate stairs and various dwelling conditions. Availability to work 10 hours per week. Reliable transportation that enables travel to client and/or patient sites within a designated area. Preferred Qualifications 1+ year of hands‑on post‑grad experience within long‑term care. Understanding of geriatrics and chronic illness. Understanding of advanced illness and end‑of‑life discussions. Proficient computer skills for documenting medical information with written and electronic medical records. Proven ability to develop and maintain positive customer relationships. Proven adaptability to change. Eligibility and Compensation New York residents only: the hourly range for this role is $42.31 to $83.27 per hour, based on local labor market, education, experience, and certifications.
