Clinic Scheduler

This individual will serve as a liaison between participant and medical staff. The Clinic Scheduler is responsible for greeting and directing participants, checking participants in and out of the clinic, answering the telephone, data entry of patient demographic information, scheduling and confirming clinic appointments and other administrative duties in support of the organization’s operations. Essential Job Functions Collaborate with care team members specifically the Primary Care Physician (PCP) and Registered Nurse Case Manager (RN CM) for appropriately scheduling and rescheduling participants’ clinic appointments. Schedule participants’ assessments. Answer the telephone by the third ring in a friendly, professional manner. Assist with coordination of day‑to‑day front office check‑in/out operations. Use customer service principles and techniques to assist participants, physicians and medical office staff in a professional, calm and pleasant manner. Confirm and document participants’ appointments in the Electronic Health Record (EHR). Ensure participants are aware of upcoming appointments and are reminded on a regular basis to ensure appointments are kept or rescheduled in a timely manner. Identify issues that may prevent a participant from attending scheduled appointments and communicate solutions to management and the participant. Review schedules, identify challenges, and develop a plan for resolution. Pull files, determine multi‑level scheduling needs for maximum participant engagement and satisfaction. Ensure necessary equipment or supplies are available for participants’ scheduled appointments. Protect participant confidentiality and rights to safeguard business and participant information. Maintain thorough knowledge of current concepts and practices of health and social services to the elderly, particularly of home‑ and community‑based services within an interdisciplinary team facilitation model. Serve as an advocate for risk‑based long‑term care programs, specifically the PACE model. Work effectively and harmoniously with staff, elderly, public and private agencies, governmental officials, service providers, and peers. Be energetic, well‑organized, dependable, flexible and resourceful. Demonstrate effective oral and written communication skills. Attend and participate in staff meetings, in‑services, projects, and committees as assigned. Adhere to and support the center’s practices, procedures, and policies including assigned break times and attendance. Accept assigned duties in a cooperative manner and perform all related duties as assigned. Be flexible with the schedule of hours worked. May be required to use a personal vehicle, if applicable. A valid California Driver’s License is required for vehicle use. Qualifications Proficient knowledge of computer skills, including MS Office (Word, Excel, Access, PowerPoint, and Outlook). Knowledge of general office procedures, equipment and filing systems. Effective oral and written communication skills. Ability to quickly learn department policies, procedures, goals and services. Attention to detail and accuracy. Ability to change priorities regularly. Experience Minimum two (2) years of experience in an appointment scheduling position, preferably in a medical office setting. Minimum one (1) year of documented experience working with a frail or elderly population. Knowledge of medical terminology. Education and Certification High school diploma or equivalent required. Graduation from a medical office assistant training program is preferred. CPR with First‑Aid certification. Must be medically cleared for communicable diseases and have all immunizations up‑to‑date before engaging in direct participant contact. Working Conditions and Physical Demands Must access all areas of the center during the workday. Must lift up to 35 pounds occasionally, 15 pounds frequently, and 7 pounds constantly; must obtain assistance when lifting or transporting objects over 50 pounds. Requires frequent hand grasp and finger dexterity; frequent sitting, standing, walking, bending, reaching forward and overhead, squatting, and kneeling. Must communicate verbally with excellent understanding of the English language. Work is performed in an indoor, well‑lit, well‑ventilated, heated, and air‑conditioned environment.
