Physical Therapist - PACE Program

Compensation

Salary Range: $105,000 - $112,000 per year

Location: Fresno, CA
Onsite

Position Summary
As a Physical Therapist within a PACE (Program of All-Inclusive Care for the Elderly) program, you will support older adult participants by delivering individualized and group therapy services designed to improve mobility, independence, and quality of life. You will collaborate with an interdisciplinary care team to assess needs, implement treatment plans, and ensure participants receive comprehensive, coordinated care in a supportive clinical environment.

Responsibilities

• Shift details: Day shift
• Hours per week: 40

? Key Duties:

• Evaluate participants' physical condition and functional abilities to develop personalized care plans
• Develop and implement individualized and group therapeutic exercise programs
• Educate participants and families on therapy goals, techniques, and rehabilitation plans
• Collaborate with interdisciplinary team members to coordinate and optimize care delivery
• Conduct home visits and supervise aides while ensuring quality of care meets established standards

Requirements

? Education

• Required: Bachelor's degree in Physical Therapy (pre-2000) or Master's degree (post-2000)

? Licenses & Certifications

• Required: Current Physical Therapist license in the state of California

? Skills:

• Strong clinical knowledge of geriatric care and rehabilitation practices
• Excellent communication and patient education skills
• Ability to work collaboratively within interdisciplinary healthcare teams
• Strong organizational and time management abilities
