Overview: The purpose of mental health care coordination services is to engage priority individuals (recipients) and their natural supports who are not yet effectively connected with services to ensure service linkage, to enhance the delivery of treatment services and recovery supports and to improve outcomes among priority populations. Priority populations include: Adults with SMI, SUD, or co-occurring disorders who demonstrate high utilization of acute care services, including CSU, inpatient and detox services. High utilization is defined as adults with 3 or more acute care admissions within 180 days; or, Adults with acute care admissions that last 16 days or longer. The Care Coordinator will be responsible for coordination of services, supports, and cross system collaboration to ensure the individual's needs are met. ESSENTIAL/CORE JOB-SPECIFIC DUTIES AND RESPONSIBILITIES- Utilize the LOCUS level of care tool and assessments to identify service needs and choices of the individual served.
- Bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, health science, or a related human services field.
- One year full time or equivalent experience working with adults experiencing serious mental illness preferred.
- Other Bachelor's degree from an accredited university or college with three years full-time or equivalent experience working with adults experiencing serious mental illness preferred.
- Current valid driver's license.
- Maintain Apalachee Designated Auto Driver status.
