Founded in 1987, Damien Center is Indiana’s oldest and largest AIDS service organization (ASO) and serves more than 8,000 individuals living with or at risk for HIV through a comprehensive, innovative approach to care and prevention. Our purpose is to be a trusted partner in providing services, education and advocacy for all people living with or at risk for HIV and any person seeking a safe and welcoming home for care. Our services include care coordination, clinical and pharmacy services, mental health, housing, and nutrition. 

Position Summary:

Working as part of the PrEP Navigation Team, the Bilingual PrEP Navigator will prioritize outreach, engagement, and care coordination for Latinx and Hispanic clients, with a focus on Latinx men who have sex with men (MSM) who may benefit from comprehensive HIV prevention services. The Navigator will develop individualized plans of care based on each client’s medical, psychosocial, financial, and supportive service needs. This may include connecting clients to health care coverage, medication access, PrEP appointments, HIV/STI testing, behavioral health services, housing or transportation resources, and other community-based supports. Through culturally responsive and bilingual services, this position helps ensure clients can access affirming, client-centered prevention care that supports their overall health and well-being.

Duties and Responsibilities:

This job description describes the general nature and level of work performed by employees assigned to this position. It should not be construed as an exhaustive list of all required duties, responsibilities, and skills. 

  • Manage an active caseload of PrEP patients.
  • Support referrals seeking linkage to PrEP, PEP, STI testing, and other medical services.
  • Assess patients for service eligibility, barriers to care, medication access, and treatment adherence.
  • Promote continuity of care and self-sufficiency by developing care plans and interventions that address patient needs.
  • Assist the Financial Counselor team with collecting documents needed for insurance enrollment.
  • Refer patients to medical, optical, dental, mental health, substance use, legal, transportation, food, and other community resources as needed.
  • Educate patients and community members about HIV prevention and treatment.
  • Complete required documentation and enter case notes within 24 hours of patient interaction.
  • Return phone calls within 48 hours.
  • Advocate for patients and serve as a liaison with clinical, prevention, and pharmacy staff.
  • Follow Damien Center and funder policies and standards.
  • Conduct face-to-face intakes and review assessments.
  • Provide patient-centered care.
  • Assist in the promotion of the program at local health fairs and events, as well as implement other recruitment strategies for the program.  
  • Collaborate with the care team to ensure comprehensive patient support.
  • Follow up on medical and other referrals.
  • Monitor patient engagement in HIV prevention care.
  • Arrange transportation to appointments as needed.
  • Perform special assignments and other duties as needed.

Education and/or Experience:

High school diploma or equivalent required. Experience with Medicare/ Medicaid and commercial Insurance preferred but not required. Experience with care coordination or patient management experience required. Bilingual in Spanish/English required. Experience working with the focus population is preferred.

Knowledge, Skills, and Abilities:

Possess superior organizational and administrative skills. Excellent interpersonal and communication abilities. Excellent computer skills in a Microsoft Windows environment. Ability to work independently and achieve high standards of productivity, achieving deadlines and with superior product results. Strong calendaring, organizational, and document management skills required. Proactive approach to solving problems and communicating to leadership. Knowledge of office management systems, procedures, and office equipment required. Excellent time management skills and ability to prioritize work.

FLSA Status: Hourly, Full-time, Non-Exempt                 

Leader: Bilingual PrEP Program Manager

Salary: $25.03 hourly
