Home-Based Cardiac Rehab Coordinator

To support the Home-Based Cardiac Rehabilitation Service, the full-time Operations Coordinator will manage patient data, coordinate with external practices and vendors, and ensure smooth administrative and billing workflows in a remote setting. Key Responsibilities Proactively track and retrieve missing clinical documentation for patient referrals across multiple EHR systems Compile, analyze, and distribute routine Key Performance Indicators (KPIs) to participating medical practices Review and monitor program data from third-party vendors to ensure accuracy and quality control Required Qualifications Hands-on experience navigating and retrieving patient records from NextGen, eClinicalWorks, and AthenaOne 2-4 years of administrative, operational, or coordinator experience in a healthcare or clinical setting Strong familiarity with electronic document management tools, specifically DocuSign, and data tools like Excel and Google Sheets Exceptional organizational skills with a proven ability to manage financial documentation Excellent written and verbal communication skills for professional outreach to external practices and physicians
